Deep sternal wound infection (mediastinitis) occurs in 1-2% post-sternotomy. Signs: sternal instability, drainage, fever, leukocytosis, sternal dehiscence. Risk factors: obesity, diabetes, bilateral IMA grafts, prolonged ventilation. Requires surgical debridement, IV antibiotics, possible flap closure. High morbidity/mortality.
